Registered Nurse (RN) – Per Diem Medicare Home Health | OASIS Experience
Dedicated Home Health is growing and is seeking experienced Registered Psych Nurses (RNs) to join our Medicare Home Health team on a per-diem basis.

What We're Looking For
Medicare Home Health and OASIS experience are strongly preferred.
Qualified candidates should have:
- Current, unrestricted Florida Registered Nurse (RN) license
- Medicare-certified Home Health experience
- Hands-on OASIS assessment experience
- Must have Psych experience
- Experience completing Start of Care (SOC), Resumption of Care (ROC), Recertification and Discharge visits
- Strong head-to-toe assessment and clinical documentation skills
- Understanding of Medicare home health requirements and plan-of-care coordination
- Ability to communicate effectively with physicians and the interdisciplinary care team
- Reliable transportation and ability to travel within the assigned territory
- Commitment to timely documentation and high-quality patient care
The Role
As a
Per Diem Home Health Registered Nurse , you will provide skilled nursing care to patients in their homes while coordinating care with physicians, therapists and other members of the interdisciplinary team.
Responsibilities may include:
- Completing comprehensive OASIS assessments
- Start of Care and other skilled nursing visits
- Medication reconciliation and patient/caregiver education
- Disease-process and chronic-condition management
- Wound assessment and treatment, when ordered
- Monitoring changes in patient condition
- Communicating significant findings to physicians and the clinical team
- Updating and coordinating the patient’s plan of care
- Completing accurate and timely clinical documentation
